Several options with deductibles of $750 or $1,500 and co-pays of $35 to $40 are available to best meet your needs and the needs of your family. Coverage is effective the first of the month following 30 days of service. Open Enrollment occurs at the end of each calendar year with an effective date of January 1.

The amount you pay per pay period for your health insurance is dependent upon the plan you choose as well as family size. All premiums that are deducted from your paycheck are pre-tax.

Dental Insurance

Employees that are scheduled and work at least 60 hours per pay period are eligible to enroll in dental insurance through Delta Dental. Base coverage includes 100% preventative care and up to $1,500 in necessary dental services. Coverage is effective the first of the month following 30 days of service.

Life Insurance and Long Term Disability

Employees that are scheduled and work at least 60 hours per pay period are eligible to enroll in life insurance, accidental death and dismemberment insurance equal to one time your annual base wage (to a maximum of $50,000), free of charge. Voluntary supplemental options for life and long-term disability are available at reasonable rates.

Flexible Spending Accounts – Health Care and Dependent Care Reimbursement

Employees are eligible to participate in pre-tax health care and/or dependent care spending accounts. These pre-tax savings are used to pay bills that you would normally pay on an after-tax basis. Your savings would be the cost of federal, state and FICA taxes. These accounts are set up on an annual basis to be used for bills not paid by any other source (insurance), during the period of coverage. Any money not used in these accounts will be forfeited at the end of the calendar year.

Retirement Savings – 401(k)

Our plan offers automatic enrollment which means all new hires age 21 and over are automatically enrolled in the plan approximately 30 days after hire. Unless action is taken, you will be enrolled at a contribution rate of 2% of your wage. You may increase your contribution at any time and may put up to $20,500 or 50% of your annual earnings (whichever is less), before taxes, into the plan.

There is no minimum hour requirement to be eligible to participate in the plan. Employees will be eligible to receive a matching contribution after one year of service of 50% up to 6% of your contribution.

Tuition Reimbursement

Employees who have successfully completed their first year of employment and are scheduled to work a minimum of 10 hours each week are encouraged to continue their education with the financial support of MMH through Tuition Reimbursement.

Holidays

Seven holidays are recognized throughout the calendar year: New Years Day, Easter, Memorial Day, Independence Day (July 4th), Labor Day, Thanksgiving and Christmas.
